Healthy Minds-Healthy Aging:
Healthy Minds-Healthy Aging is a prevention and early intervention program for residents of Napa Valley who are 60 years and older, and show early signs of depression and/or cognitive decline. We provide free, in-home bilingual services, which include evidence based assessments, case management and brief therapeutic interventions.
In addition, Healthy Minds-Healthy Aging provides community education regarding dementia and depression to reduce stigma and encourage access to therapy.
Our focus is on emerging mental health and cognitive impairment issues among older adults and their caregivers, and as such we believe that we:
- Enhance the quality of life for older adults and their families by enabling them to live as independently as possible for as long as possible.
- Improve access to and utilization of services for mental and cognitive health concerns.
- Increase early identification of mental health needs and cognitive decline.
- Increase capacity and improve coordination among providers.
Healthy Minds-Healthy Aging Program is partnership of Mentis: Napa’s Center for Mental Health Services, Collabria Care, QVMC CARE Network and Napa County Comprehensive Services for Older Adults.
